Process Safety Lead - Incident Investigation

Highly successful oil and gas exploration and production company.

The Role

Our client is seeking an experienced Process Safety Specialist to implement the company's Asset Integrity - Process Safety investigation requirements and provide support with:
  • Classification of reported Asset Integrity - Process Safety incidents
  • Facilitation of the investigation process (TRIPOD)
  • Assurance of delivery of quality investigations and comprehensive investigation reports
  • Incident analysis and feedback on all Asset Integrity - Process Safety incidents to influence future company strategy
  • Developing a network of competent Asset Integrity - Process Safety investigators.

Key deliverables and challenges for this role include:

  • Improve and sustain the quality and timeliness of Asset Integrity - Process Safety Tier 1 investigations
  • Improve the analysis of Asset Integrity - Process Safety incidents and near misses
  • Influence at all levels of the company to enable behavioural and organizational change
  • Identify and recruit internally, suitable candidates with the potential to develop into competent Process Safety Incident investigators and Tripod Beta Practitioners
  • Embed the culture of openness in relation to the reporting of Asset Integrity - Process Safety incidents
  • Investigate previous Tier 1 Incidents that were inadequately investigated
  • Use learnings from incidents to influence future Asset Integrity - Process Safety strategy.

The Person

The following skills, experience and qualifications are required:

  • Degree or higher in Chemical Engineering or other relevant discipline
  • Minimum 8 years experience in Operations/Maintenance/HSSE in the Oil and Gas sector (upstream and midstream), with a minimum of 4 years experience in EP incident investigation and analysis
  • Certified in Tripod Beta incident investigation techniques, and experience with other investigation techniques such as Taproot, Event Trees and Root cause assessments, Bow Tie methodology
  • Sound knowledge of the CCPS Asset Integrity Process Safety Management protocols
  • Knowledge of HSE management systems, the HSE management of contractors, and Human Factors and Behaviours
  • Proficient in the use of IT tools such as MS word, Powerpoint, Excel, Fountain Incident Management system, Bowtie-XP
  • Proven effective interviewer of personnel to gather incident data, sound analytical skills and ability to write clear English reports
  • Proven effective communicator capable of influencing at all levels within the organization.
  • Proven coaching and mentoring skills for junior staff
  • Strong project management skills
  • Highly motivated
  • Effective as both a team player and team leader.
